Does anyone have Fiserv Health (PHCS) if so do they pay for the lab band

The best (and easiest) way to find out if your insurance covers the surgery is to call the number on the back of your insurance card and ask. Even someone who has PHCS surgery could have a totally different policy since each employer negotiates their own contract for insurance coverage. Hope that helps, Pam
