HELP- concerning approval-

Does anyone know about Blue Cross health mate and the requirements for five years obesiity? I was obese my whole life obese and I don't have records DOCUMENTING MY ACTUAL WEIGHT for five years- (I can show three years of obesity) but I can provide health risks like high blood pressure? can someone help? I have high blood pressure- hypothyroidism- shortness of breath- -sleep apnea-BMI 40- do you have to provide medical records concerning weight documentation for five years-or medical records documenting your weight? or would the health issues override it?
