I'm so sorry that I didn't see your message until now! I did use Tricare for my surgery. I did not have to provide a 5 year medical history. My PCM referred me at the end of December to a Bariatric Surgeon and it was approved immediately (for the doctors visits only). I was set up for a seminar in March along with my first appt with the surgeon. The doctor asked me to lose 5% of my body weight, get an EKG, Psych Eval, attend a minimum of 3 support groups and once that was done they would submit the authorization for the surgery itself. They did this around the middle of May 2008. I kept checking tricare online and my authorization kept saying "pending." On June 1st when I saw the surgeon again they told me I was approved and scheduled for July 28th. That was great news except my co-worker was on vacation on August 1st so I had to postpone it until the next available date.
My BMI was (and is) over 50 and my co-morbidity wasn't listed on the tricare website (High Cholesterol) but they approved me.

Thank you. I apologize for the late response. Your reply helped. I hope it does not take six months. I hope to have the surgery before Christmas. i do not need the referral and I do have the medical history. I have an appoint with the doctor Wednesday, Sept. 3. I plan to attend my first support group the first Tuesday in September. I weighed my self Tuesday, and my BMI is 50. We were told at the seminar about the Psych Eval and other test we may have to get done. I was told that the doctor may let me get some them done at VA. I will let you know how the doctor's visit goes.
