Diet Records

I was reading a post that someone wrote about diet records that I could possibly get online. I have been doing WW for 5 years off and on and have no real records except for the diaries I have kept myself. Could someone please forward a copy of a letter that my doctor could sign. He isn't very thrilled about my interest in LapBand since he has a weight loss clinic in his office. I just do not want to be on diet drugs because I have a heart condition. Anyway, any time of help is welcome. I would really love to get this done. Thanks!!
