Medical alert ID's

I know we've gone over this but I have slept since then and I am having a hard time finding the post. I really want one of these for myself but need to know what I should put on it or how to word it. If any of you have one and want to give me some ideals i'd appreciate it.

Mine says: My name, Gas. Bypass, NoBlind NG, No Nsaids, and PCN (Allergy to Penicillin). This is the one I got: http://www.creativemedicalid.com/inc/sdetail/1209 and you can add charms to them, I just added a cross and got a pink/black charm med charm.
