This is great! I had to do a trade show with restaurant eating last week. I ate the same meal you did (more or less) BUT I didn't know about the restaurant cards and had to pay full price. I was happy that I enjoyed my dining experience, much more than I thought I would in fact, but I'll print out the card before it happens again.

No, you don't need to have your surgeon sign it. Just write in his name, and whatever else you're supposed to write in (his office phone #), and then go ahead and use it. I actually made 6 different cards, with the same message but different borders, and had them laminated at Staples. I laminated them all on one sheet then cut up the sheet to have 6 individual cards. That way, if I lose one, I have back-ups already in place. Plus, laminating them will make them last longer. Don't forget to ask for the card back after showing it to the waitress/waiter or manager.
