I've heard that many surgeons and bariatric centers have these cards. I don't use one since I don't eat at buffets anymore. And the few times I have, I just gave the rest to my hubby. I don't find it a real issue. Many places will also just let you eat of the child's menu, but I've found that those items aren't what I should be eating anyway so I just go for what I want and take the rest home.
As far as the century club, I think you need to e-mail the folks in charge of profiles, but I don't know. I'm not there yet.

FYI I also found no use for the restaurant cards...I'm just in the habit now of either taking extra home or leaving it. The kids menu still usually has more than I would eat and not the type of food I would eat...the Seniors menu is again still more than I would eat. And I don't mind leftovers. And most importantly I've given up the idea that I have to get my every pennies worth used. I was raised to take seconds and thirds and ALWAYS finish my plate. I don't think that way any more at all. If it's a type of food that doesn't reheat well...I leave it.
