Had lap band approved by Medicaid in TX? Example letter Plz.

I have a patient with a BMI of 57 who is on Medicaid and desiring lap band surgery in Fort Worth. If you have been approved for lap band through Medicaid, I would appreciate a copy of the letter(s) used to request the surgery.    — Stephanie B. (posted on November 18, 2002)

yes, as of jan 2009 it is now approved and will be covered .. ia m still looking for a doctor though
