are there any sample appeal letters for breast lift

I dont have a sample; but what I did was go online and look at the
requirements my insurance was requiring and then submitted everything they
wanted..

You will have better luck if you request a breast reduction. Breast lift is
considered cosmetic only! Most insurance companies have requirements about
how much volume or weight of tissue has to be removed to qualify as a
reduction. Be sure you have pictures of your breasts and any indentations
in your shoulders. Document shoulder and neck pain and frequent headaches.

Hi. I agree with the first poster...a breast lift is much harder to get
covered. A breast reduction does require the removal of a certain amount
of tissue, so if you are small, you may not be a candidate. A lift MAY be
covered if you can prove it causes skin rashes and document them. REMEMBER
there is no "magic letter," you need to find out WHY you were
denied and appeal only after you have met the INC company's requirements.
