DO YOU HAVE TO SHOW PROOF?

Alicia, The only way to answer your question is to find out what your
insurance requires. Even if someone else has the same insurance company
you do, their employer may have different rules or even a different kind of
policy. Your HR department should be able to get a copy of the policy for
you, or you can look in the book you should have received with the details
of what your insurance covers and what their requirements are. Some don't
require any proof of physician-supervised weight loss attempts. Some want
1 year's worth and others more than that.

I have United Healthcare too, for me they were awesome they didn't ask for
any kinds of proof, and my acceptance was fairly quickly. I am not sure if
you are on the same plan as I am or not. I suggest calling them at their
customer service number, they were very easy to talk to, and cut right to
the chase with me, and got me approved. Judi

Called HR they knew nothing called UnitedHC and spoke with a care
coordinator. And she said that I would only need the information from the
surgeon's office, and I'm assuming that is the psy test, sleep test, etc.
