Question:
Why has my dosage dropped?
When I started on Coumadin and my dose stabalized I was alternating 10 and 12.5 mgs daily. This was in 1995 when I weighed 325. Then in 2005, at 500 pounds, I am still taking the same dose. Now, post op, my dose almost immediately needed to be reduced and am currently taking 5mgs daily. It obviously isn't weight related because I was much lighter taking a higher dose than I am now? Has anybody had this explained to them??? — David B. (posted on June 27, 2005)

Hey David,
Coumadin is not a weight based drug It is titrated to a lab value called
pt/inr. The dosage is then given according to the level of the pt/inr and
what your md wants it at. Also your dietary needs play a part in the level.
Avoid vit K because this is the antidote and will reverse your level too
low. Also you have to be careful with green leafy vegs. I am sure you know
your dietary restrictions. But understand it isn't based on how much you
weigh.

Thanks Deb...I understand all that although I forget about the leafy vegies
sometimes. My only question is this...what has changed so drastically
after surgery that affects the absorption of coumadin. Thanks again!

David, were you on any herprin or lovenox shots following surgery? for how
long? those shots are blood thinners as well , if you were on them for a
few weeks that could also explain why you does was lowered. it is determind
by your blood levels (PT/PTT)how you doing otherwise? Tracey
