Question:
Desperate to find actual copies of appeal letters for Post-op plastic surgeries.

I have BCBS (Federal Employees Benefit Plan) and they paid for my WLS surgery in December 1999 with no problems at all. I am trying to have an extended abdominplasty or sometimes called the beltlipectomy, to removed and fix the leftover "mess". I met with a plastic surgeon for consultation and he wrote a letter to my insurance company and it was denied twice. His basis was the rashes under the apron, etc. I have a short window of time to work on an appeal at the Federal Level of this and am looking for copies of actual appeal letters that were used, from various resources such as PCP, chiropractors, etc. so that I may do this in a professional manner.
