Question:
How do I find a sample appeal letter for a TT?

When I originally applied for my gastric pass there was a section on obesity.com that had samply appeal letters. I read over a few of them, wrote my own letter and took it into my PCP. I was apporoved for surgery very quickly. Now I am trying to get Tri-Care apporval for the removal of my excess stomach skin. The PA in my PCP refuses to write me a referral. I want to go talk to patient rep. next week. I am taking photos of the excess skin and want to take a letter with me. Does anyone know where or how to find the section with the appeal letters?
