Question:
Does any on have a letter from their orthopaedic dr

I have a orthopaedic surgeon that will write a letter for me but wants to see a sample letter. I have upper and lower back pain,and a hip injury that will eventually need a hip replacement. I am just looking for a sample letter from a dr so my dr can get a idea on how to write one. Can anyone help?    — michellemark2764 (posted on September 16, 2002)

Michelle, I didn't save a copy of my letter from my doc but it was very informal. My PCP simply wrote long hand on a script pad something to the effect "I recommend bariatric surgery for this patient, she is unable to exercise due to orthopedic trouble". I was approved on my first attempt. So far I've lost 72 lbs and even after only losing around 30 lbs in the first month it made a remarkable difference in the amount of joint pain I had.
