Question:
My PCP has never done a med. ness. letter

HELP! My PCP has never wrote a Med. Ness for this proceedure & would like some sample letters so she can do her best. Dose anyone know where I can find some? Thanks Loads    — Sabrina N. (posted on June 7, 2002)

Hi, I don't have a copy of the letter with me, but my PCP just stated my comobitities and threw in that due to comorbitities and familiy history of heart disease, stroke and diabetes, it is necessary for me to lose weight. Also, I've tried other conventional weight loss methods both medically supervised and on my own with little to no success. The letter was accepted without a problem.
