Question:
What does the doctor write for medical leave
First of all I haven't told my work place about the WLS, I told them I may have to have my gallbladder taken out. So today I was given a form for my doctor to fill out. One question on the form is " explaination of extent to which employee is unable to perform the functions of his or her job" and another one " is the employee unable to perform work of ANY kind."I have to return this form to my office manager and she sends it to corp. office.... Now what do I do? I don't want to lie, but then again I don't want them to know the truth, it my personal choice. I don't want any one at my work to know the real reason, because they won't understand, or may not approve of me taking the time off, and I have to keep my job.... HELP.... — lyn K. My doctor wrote that I have been under his care since 3-28-01 and I would
be able to rerun to work on April 30, 2001. No details, just the facts.

My note said I was scheduled for abdominal surgery.

On my form they simply wrote 'surgery', said I cannot perform any
activities related to my job, and put my return to work date at 6wks
post-op at my request (I think he would have let me go back earlier since I
have a desk job). However, the HR guy called me after he received the form
and said they needed to know what type of surgery I was having. I said I
was having my gall bladder removed (the truth) and also having some stomach
surgery done at the same time. He was satisfied with that. If you are not
having the gall bladder removed you could try saying 'stomach surgery'
which is pretty generic and that might get you by. Good luck.
