Question:
does your employer let you put in medical leave papers for surgery?

Yes. I met with my HR disability rep. She was helpful. Since I just was
approved, I need to go back and give her my operation info. She will then
do the filing for me. You should go ASAP before your surgery since it takes
time to set it up. So, WLS meets the criteria for short term disability.

I was able to use FMLA and STD disability. It depends on if your company
has it based the company size. However, as long as it is for health
reasons, it should be. Check with your HR FMLA person.

If you have been at your job for at least 12 months and have worked at
least 1250 hours, you should qualify for FMLA. Meaning your job is
protected for up to 12 weeks. However, this time does not always have to
be paid, each company is different. If you have enough sick and/or
vacation time to cover your time out then of course it would be paid. As
long as the proper documentation is submitted in a timely manner than they
should approve you. It's best to talk to HR before you go out to see if
any particular forms need to be filled out prior to going out for surgery.
Some doctors do require fees for filing out forms so just be warned. Even
if you don't qualify for FMLA if you have Short Term Disability, then they
should cover your period of being out. Again each company has it's own
rules about using different things, sometimes, STD kicks in after all sick
time has been exhausted, which if you have enough sick time accrued the STD
is useless unless you have major surgery keeping you out 6 weeks or more.

Check with the Human Resource person at your work she/he should beable to
tell you what you need to do to get medical leave. I work in education and
have a 10 month position, however my medical leave insurance covers me for
12 months and I will be able to get coverage when I have surgery in June
and get payment eventhough I'm not working.

yes they do. just submitted mine this morning. or you could always have
your surgeons office fax them over for you.

Yes, I had to file FMLA papers with the HR rep.
