What version of BCBS? Revision from what to what? And why? If your original surgery is broken, for example, it's easier than a flimsier reason. They decide "flimsy", not me.

I have the same question and am going through the preop process now. I have a SLD-gastrogastric fistula and BCBC of MN - although they only act as a TPA my employer is self insured.
