i have BCBSNJ and i was approved with no problems. i didn't have to pay
ANYTHING at all (aside from copays at the office visits). you should find
a center that has an "insurance specialist" on site because THEY
will file all the paperwork and know how to get it done. you have to jump
through some hoops and possibly wait 6-7 months. but for me it was SOOO
worth it!! good luck!!!!

Check with your insurance carrier and find out what their requirements are.
Some want a 6 month medically supervised weight loss program, psychological
evaluation, nutritional eval. Every insurance company has their own
requirements.

I have federal BCBS and their own requirement was a BMI over 40 or a BMI of
35 with co-morbidity. I just called the number on my insurance card to get
the information and made sure to take notes while I was on the phone and
get the name of the person I was speaking to.
