Pannilectomy Letters: Does anyone have any Pannilectomy letters of appeal?

Hi, trying to fight for panni to get done. Does anyone have any pannilectomy appeal letters. I have the abdominalplasty letter, but didn't think I could use that one , since my doc submitted my paperwork to insurance company as Hernia repair with pannilectomy.
