Question:
I need to appeal my denial to my insurance co and employer (they are one in the same)

I work for BlueCross Blue Sheild of Oregon and my insurance is BCBS HMO Oregon. I have a clear contract exclusion as was just informed today my surgery auth was denied (which I expected.. yet it still stung rather badly!). I want to get an appeal letter off asap.. and I want to write something up to my human resources/benefits person as well. If you have anything I can use that you are willing to share I would greatly appreciate it. I want this more than I have wanted anything else in my whole life.
