Need help with appeal letter to insurance.

I have looked at all the answers in the library, but most of the information is not current. My insurance is Tricare Prime. I am 18 months out and am requesting a consult to a plastic surgeon for a panniculectomy. The denial states: This determination of services is not covered under Tricar/CHAMPUS policy is based on the following: Services r/t gastric bypass surgery r/t obesity or weight reduction wheter surgical or non surgical treatment are uncovered.
