Has anyone had the Lap Band surgery? Surgeon and Insurance?

When I decided to have the weight loss surgery, I decided to go with the Lap Band. Is this type of procedure something not many surgeons will do? and how about insurance coverage...I have BC/BS Traditional (GM) Has anyone with this type of insurance had the Lap Band surgery covered?
