Switching surgeons and approval process

Florida medicaid is now requesting (preferrably) nude pictures as part of the approval process, has anyone come across this before? I have come across some information about the surgeon I have seen and who is working on the approval for me that makes me very uncomfortable to let him proceed with the surgery. However, I am desperate for this surgery. If I found another surgeon who accepts medicaid would they be able to take over from the first surgeon, if approval is recieved by the first surgeon could they use that approval or would I have to start all over again, tests, etc?
