Intracorp is requesting a year of documentation from Dr and Nutrionist evaluation

I may be able to come up with 6 months when I was on Phen Phen and the rest of the time I did home items Diet Center and Nutasytem without Drs documentation. What do I need to expect? I am worried now.Is it over for me? Wanting this surgery.
