How does medical insurance differ between a abdomenplasty and a lower body lift?

I know letters of medical necessity(rashes and back pain) are helpful for a tummy tuck. What does approval for a lower body lift require. I realize all insurances are different and nothing is a guarantee but I could use some guidance regarding this. Thank you
