Anyone know of an Individual Policy in TX that covers WLS?

My group Health Insurance Blue Cross Blue Sheild of Texas (Notorious for this kind of thing) will not cover WLS (exclusion). I also have a Hiatal Hernia that causes me reflux so bad I sometimes come up with a mouthful of vomit. My Dr. said I can get the WLS and Hernia corrected at the same time, but BCBS says that it is not "medically necessary". I have read my policy diligently and could not believe that the definition of "medically necessary" could be determined by the insurance company! Isn't this up to the doctor and me? I don't want to go through life burping up vomit in front of people! We are fighting it, but I know how they are.
