Why has my dosage dropped?

When I started on Coumadin and my dose stabalized I was alternating 10 and 12.5 mgs daily. This was in 1995 when I weighed 325. Then in 2005, at 500 pounds, I am still taking the same dose. Now, post op, my dose almost immediately needed to be reduced and am currently taking 5mgs daily. It obviously isn't weight related because I was much lighter taking a higher dose than I am now? Has anybody had this explained to them???
