How much does a breast lift cost? Are there any ways to get ins. to pay for it?

I am down 85 pounds and my boobies are pretty saggy. I think the only "plastic" surgery I may want/need to have is a breast lift. I have BCBS of Minnesota PPO. Does anyone know if they cover it? How much does a breast lift cost on average? Is it really painful?
