Any dr's who take self-pay low up front payment &hospital that goes based on income?

HI I was just wondering. Are there any surgeons that have reasonably low up front fees and that work with hospitals that will work with someone who cannot get financing based on income??? I am looking for surgeons in any state. I am not picky. I am trying to help out a friend.
