BCBSof IL. PPO denied my TT

I'm 1yr out this month and have hernia's that are really bothering me. My bypass sur. said for me to try to get a tummy tuck at the same time. I've been at the same weight since Dec. 04 and only 9lbs. away from my personal goal. So I went to plastic surgeon and he agreed to do a tuck and some lipo. Anyway today I was denied. Can anyone tell me what to do or email me on how to appeal (letter). I'm at a lost and very upset. I was overweight 20 yrs. so skin in abdom. hardly shrunk at all. Haven't had rashes yet but as weather gets warmer I probably will. I am having back, neck and shoulder pain also. Please help! Thanks in advance.
