KEYSTONE EAST INSURANCE DENIES ME TWICE

I was denied twice for a breast lift from keystone east and was wondering if anyone else has this insurance and what I have to do to get it approved. I even had 2 doctor notes from my primary physician and 1 from the surgeon. HELP!!!! I am worried my tummy tuck won't be covered either!
