Premera Blue Cross says they have an exclusion. How do we go about it to be accepted

They are my primary, I think; my husband's work insurance. My secondary is Medicare, Parts A and B. If Premera won't do it, does Medicare ever accept wls and under what circumstances and how do I go about getting it covered?
