Does anyone have copy of the letters PCP & Psychiatrist need to sign/ write?

HELP! ASAP, PLEASE! I need help, I have an appt. with my PCP on 12-26. She told me to bring the necessary letter to her, at this appt., & she will sign it & mail it in, or mail it to me. I have no idea what the letter should say for my acceptance with WLS. PLease, if anyone has a copy (of their letter)that they could e-mail me at [email protected], I would really appreciate. Also, the same thing goes for the Psychiatrist's letter. I have an appt. with him on 1-2 & he told me to bring the letter for him to sign. As you see, I need help with getting these letters ASAP!
