I need to know how to go about getting approval for a panni

Open RNY 11-12-01 loss -105 and I am ready to try to get a panni done, but I was forced to change insurance on 1-1-02 and don't know how to go about getting approval for the panni with this company (which by the way does not cover anything for WLS surgery or TT) so any suggestions will help. I have not seen a cosmetic surgeon yet, So is there any way I can apply for approval and try to get it before I see the cosmetic surgeon?
