I have CIGNA HMO, need help with an APPEAL LETTER for WLS

I'm needing samples of APPEAL LETTERS FOR WLS. I have CIGNA HMO and understand they can be very difficult to deal with. I could have surgery by the middle of April if I can get approval from CIGNA. I want to have an appeal letter ready to send out. Thanks for your help.
