Need a copy of letter to either/orDept of insurance,HFCA,,congressman etc

In need of a good letter without curse woords but stern asking for help from Dept. of Insurance, congressman HFCA, consumer advocate etc. My insurance company has denied me twice for petty reason, and is being unprofessional and misleading. They need to know there's is not the final decision and that they are being WATCHED.
