any one have information on Dr. Poplawski?

I have surgery scheduled for Oct 15, 1999. Dr Poplawski is my surgeon, but I cant seem to find him on any of the lists of surgeons and I also can not find anyone that is or was a patient of his. If anyone could help me out i would appricate it.
