How do I get a good referral letter from my GP?

My GP wrote a referral that only said He recomended that I have the surgery. He didn't elaborate and give andy details about my specific reasons that the surgery would be beneficial. Has anyone written thier own letter and just gotten it signed by the Dr. or would I be way out of line?
