Can companies add insurance exclusions of weight loss surgery within the contract yea

We have recently been told that our employer has added an exclusion of weight loss surgery to our insurance of BCBS effective two months ago and nobody told us about it. My co-worker has a letter of approval already but the insurance company saids that if your contract doesn't cover this that insurance would not pay for the surgery.
