Can anyone show me a sample of past diet history?

I am a fairly good record keeper when it comes to bills and payments but when it comes to diets I can't begin to tell you how many "journals" I have toosed out. Now I really don't know more than just the last year or so of diets. Can anyone show me or send me a sample of what they gave their physicians to show them their diets? I am 30 and have done many a diet since I was 15 but to save my life I can't fully remember too many. My email is [email protected]. thanks in advance!!
