Copies of Appeal letters sent to TriCare Prime Insurance?

I was denied for the plastic surgery because they considered it cosmetic. I have lost 122 pounds and I feel it is necessary to have to avoid future problems with the excess skin. Please any samples of letters I could use for my appeal I would appriciate it.
