I was just denied plastic surgery

My insurance company Capital District Physicians Health Plan just called me with the awful news that my doctors request to remove the acces skin from my stomach was denied. The insurance company said it is now up to me to appeal the decision. Does anyone have any ideas of place that I could get information from?
