What qualifications need to be met for tummy tuck, arms, back, breasts, etc?

I was denied for all by my insurance carrier (PHCS, Maryland). They said I did not meet the qualifications for the plastic surgery. My panni hangs 1/3 down my thighs, I have skin on my back that hangs 1/2 way down, I have to wear clothes way too big to tuck all the skin in, I am constantly fighting rashes, it is horrible. I cannot believe they would expect me to stay like this!! Help!
