What to have engraved on it?

Would someone please tell me exactly what to have on my medical alert braclet? I had the open RNY bypass on Jan. 21 2003. My husband had the same surgery on Jan. 10, 2003 and we are both ordering one thanks to the great info we have read on this website. We think is very important that we wear one at all times.
