What are the requirements for medical necessity?

I am currently a size 48F. I have been approved for the WLS surgery. Once I have the surgery and lose the weight, I am sure that my breasts will hang much lower than they do now. If anyone has had this type of surgery approved by their insurance company, what type of documentation of medical necessity is needed?
