Anyone with BMI under 50, with no comorbids, get approval?

I am asking for my friend, and am wondering if anyone with a BMI under 50 (is 46), with no cormorbids get approval by Kaiser Southern California in 2002? Did you have to appeal and what was the process? Any help would be appreciated. Thanks.
