if supervised diets rom Dr. are from 6 yrs. ago would ins. accept this?

My doctor supervised records are from 1995-96 all the others were on my own or jenny craig or weigh watchers will ins. accept this for my 12 month supervised diets? I called ins. comp. they told me needed documentation for 12 months but didnt tell me any time period. Any info. will help me. THANX
