NYS Empire Plan..Does it cover panni?

I was wondering if anyone had NYS government employee health insurance (Empire Plan) that has had plastic surgery after WLS and if the insurance covered any/all of it. Also, what were the qualifiers if it did...ie. recurrent skin rash/infection or difficulty cleaning under skin flap.
