OK, I need copies of letters sent from PCP's please!

I had my consult with my surgeon and he is having me go for a pre-surgical physical and I will need to get one of those medical clearance/approval letters from my PCP (which she has already agreed to). To help her out, I would like to see copies of what other PCP's had written. NOT a reccommendation for referral, but for the surgery. Thanks!
